What questions does the ESTA form ask?

Personal information of the traveler

In this first part of the questionnaire, candidates are asked to fill in all their personal identity details. The main data to be entered are: first and last names, date and town of birth, gender, marital status, email address, parents’ identity, contact details of last employer.

Since December 2022, it has been necessary to download your passport for the USA online to validate the information entered in the first part of the form.

Travel information for the United States

The second part of the form on the official ESTA website allows you to enter your travel details for the U.S. territory. The traveler must provide information concerning the airline, the city of embarkation for the trip and the place of residence in the USA during the stay.

Finally, it is necessary to indicate the contact details of the person to be contacted in an emergency, whether in the United States or abroad.

Questions about health and criminal record

The last part of the questionnaire consists of 10 qualifying questions. These questions enable the U.S. authorities to determine the traveler’s eligibility to enter the U.S. under the Visa Waiver Program.
